Welcome — I’m glad you’re here

This intake helps me get to know your student and your family before we meet, so our time together can feel thoughtful, grounded, and meaningful.

A few things to know before you begin

There are no right or wrong answers here. This form is simply a starting point—an opportunity to share what feels important about your student, your family, and where you are in the college process right now.

Some questions are reflective and open-ended. You’re welcome to answer briefly or in more detail, whatever feels right for you.

  • The form takes approximately 10–15 minutes

  • You do not need to complete everything perfectly

  • You can skip any question that doesn’t feel relevant

Your responses are held with care

Everything you share is used solely to support your student and guide our work together. I review each intake personally and thoughtfully before our conversation.
